Renal dose adjuster

Automatic dose adjustment is unavailable. CKD stage alone does not establish a safe drug dose or interval. Review a creatinine range below, then use current drug-specific references and patient assessment for dosing.

Uses the IRIS table in the selected laboratory unit, without rounding your input. Values between published ranges require review. Unit equivalents are approximate (1 mg/dL = 88.4 µmol/L); the published cutoffs are rounded and do not convert exactly.

Creatinine reference only: this does not diagnose CKD or assign a final IRIS stage. Do not apply CKD staging to suspected acute kidney injury, dehydration or an unstable patient. Interpret creatinine with SDMA, muscle mass, breed/size and serial trends; substage by renal proteinuria (UP/C) and blood pressure.

Basis and limits
  • IRIS 2026, dogs: Stage 1 <1.4; Stage 2 1.4–2.8; Stage 3 2.9–5.0; Stage 4 >5.0 mg/dL.
  • IRIS 2026, cats: Stage 1 <1.6; Stage 2 1.6–2.8; Stage 3 2.9–5.0; Stage 4 >5.0 mg/dL.
  • µmol/L, dogs: Stage 1 <125; Stage 2 125–250; Stage 3 251–440; Stage 4 >440.
  • µmol/L, cats: Stage 1 <140; Stage 2 140–250; Stage 3 251–440; Stage 4 >440.
  • Precision: a value between 2.8 and 2.9 mg/dL or between 250 and 251 µmol/L is flagged for review rather than silently rounded. Near a cutoff, confirm laboratory reporting precision and interpret serial creatinine and SDMA together.
  • SDMA: persistent discordance may change the stage and treatment category. The IRIS SDMA recommendations use the IDEXX assay; equivalent results from other assays are not established.
  • Low creatinine: a Stage 1 creatinine range alone does not establish CKD. Other evidence of renal abnormality is needed.
