Basis and limits
- Adjustment basis: creatinine and IRIS stage as surrogates for glomerular filtration rate.
- Stage 1: minimal adjustment for most drugs.
- Stage 2: mild reduction (typically 75% of standard) or extended interval.
- Stage 3: moderate reduction (typically 50% of standard) with extended interval.
- Stage 4: significant reduction (typically 25% of standard) or consider alternative drugs.
